Care pathways Australia

In late 2025 Streamliners sought input from healthcare stakeholders across Australia on a proposed national approach to integrated care. 

We invited healthcare professionals and wider stakeholders in the health and care sectors to share their views on a proposed national approach to integrated, localised care pathways. In our view, a shared national framework that supports local adaption would help address fragmentation and complexity within the Australian health system and shape the future of care in Australia.   

A broad cross-section of clinicians and other health system professionals took part in the consultation process.


Independent review of findings 

In early 2026 Streamliners commissioned international management consultancy Nous Group to conduct an independent review of the consultation responses. The Nous Group report has confirmed strong, cross-sector support from clinicians, health services, policy leaders, and consumer organisations across Australia for a nationally aligned approach to care pathways.  

The collective message is clear - care pathways are vital infrastructure for delivering safer, fairer and more consistent care, but they cannot reach their full potential without stronger national direction, sustainable stewardship and better integration into everyday clinical practice.

Next steps

The consultation document Care Pathways Australia 2026-2030 provides insights into how this unified national approach could transform the way care is delivered and connect Australians to the right care, first time, every time.

Share your views by Sunday 21 December

We invite you to share your ideas in our submission form by Sunday 21 December 2025. Your input as a key stakeholder in the Australian health system is essential. We want to hear your ideas on what you think would work at a system level, drawn from your experience.

Your views will help co-design a strategy that supports equity, integration, sustainability, and digital innovation, aligned with the Quintuple Aims.

You can share as much or as little as you like – there are no compulsory questions. If you need time to think about and return to your responses, make sure you leave your submission form browser tab open, as you won’t be able to save and return to your progress in the survey later.  

We’ll keep submission responses confidential, summarise comments anonymously, and share key themes in the final outcomes. Respondent email addresses are required in case we need to contact you about your response, and to share the outcomes of the nationwide submission process.
