West Moreton HealthPathways in Queensland launches with celebration and workshop event

A combined launch and workshop event held last month has seen the HealthPathways implementation in new region West Moreton, Queensland off to a great start.  

The event, held on Tuesday 7 August at the North Ipswich Corporate Centre, was attended by over 70 clinicians, leaders and interested parties from across the local health and social care system.

Representatives from West Moreton Health, Darling Downs and West Moreton Primary Health Network (PHN) and primary healthcare professionals from throughout the region received a high-level strategic view of HealthPathways before taking part in workshop sessions.

The overall programme was designed to provide a broad overview of HealthPathways, and its benefits for local patients and the community, says Dr Kerrie Freeman, Chief Executive, West Moreton Health.

“What I find particularly exciting about the HealthPathways model is that it follows the “local is best” principle. The HealthPathways being built will be those that are relevant to this community, which means they are more likely to be successful and to be used. The HealthPathways model also puts strong local partnerships at the centre of the stage, so we can care better together."  

“HealthPathways will make a difference for healthcare clinicians in our region by providing localised referral pathways, enabling the right care at the right time and in the right place for patients,” said Ms Merrilyn Strohfeldt, CEO of Darling Downs and West Moreton PHN.

“The launch event and workshop was a wonderful opportunity to build excitement with healthcare professionals in the region about the impact HealthPathways will have through integrating models of care to deliver better health outcomes for our community.”

Dr Aaron Kennedy, Regional Clinical Advisor for Queensland, provides insight into the local context for HealthPathways.

Sara Riggs, Clinical Nurse Consultant WM HPs said plans were underway to start HealthPathways training for general practice teams and local clinicians on 9 October 2018.

“From now until then, the team will be busy service mapping both private and public services across the West Moreton region, and selecting which pathways to prioritise and localise,” Mrs Riggs said.

Andy Froggatt, Chief HealthPathways Implementation Advisor, says the event was a great success.

“It was wonderful to hear the commitment from the local health community to improving services in the region and to see that HealthPathways could play a significant role in assisting them to achieve their aims,” he says.

“HealthPathways is not only an information portal - it provides a shared platform for health practitioners throughout West Moreton to discuss their patient’s journeys and share their knowledge and experience with each other, in the process creating a better-connected and informed environment in which to provide healthcare.”
