On Wednesday 7 July we’ll present an introductory webinar about Community HealthPathways for health system professionals in the United Kingdom.
Dr David Hambleton will facilitate the webinar and introduce the concept behind HealthPathways. There will be a live demonstration of HealthPathways by Dr Helen Horton (GP Lead North Cumbria). Kate Hudson (Chief Finance Officer/Chief Officer for South Tyneside CCG) will talk about the return on investment seen in South Tyneside. The webinar will conclude with a discussion by a panel of health professionals who have HealthPathways in their health and social care systems.
After the webinar, we’ll share the video with attendees and other interested parties.

About Community HealthPathways
Community HealthPathways (CHP) provides primary care clinicians with access to a suite of locally-agreed pathways covering assessment, management, and referral to community and hospital care services.
The pathways are agreed between primary and secondary care clinicians, based on evidence-based guidance, and reflect "how we do things around here".
They facilitate informed decisions at the point of care, promote standardised ways of working, and support effective referrals to community and hospital care services.
