The tables are based on the ACT and SNSW evaluation framework (available on request).

Evaluation frameworks

• Regular Monitoring 
  For continued assessment and adjustment of programme work priorities and engagement activities. Outcomes from this framework support your reporting and can provide a basis for further evaluation. 
• Process Evaluation 
  For understanding the implementation, systems, and procedures of your HealthPathways programme, rather than its clinical impacts and outcomes.
• HealthPathways Engagement – Programme, Executive/Funders, Website
  For investigating engagement with HealthPathways, which can be difficult to assess due to the subjective nature of feedback.
• Clinical Impacts, Audits, and Improvements
  Due to the complex nature of these evaluations, we have divided them into four different frameworks:
  • Identification of System Redesigns and Clinical Improvements
  • Evaluation of System Redesigns and Clinical Improvements
  • Pathway Audits
  • Referral Audits 
• Implementation Evaluation
  For an overview of setting up an evaluation during the implementation phase of a HealthPathways programme. It is intended to be used in conjunction with the Regular Monitoring framework.
